Information for Former Patients of Dr. Simmonds

If you were previously enrolled as a patient in Dr. Simmonds' practice and have not yet completed the enrollment form for Dr. Yavari's practice, please complete the Primary Health Care New Patient Declaration Form and return it to our clinic as soon as possible to remain enrolled with our practice.

You may drop off a paper copy of the form at the clinic or call the office for assistance.

You do not need to request a transfer of your medical records. Your records will be transferred automatically and will continue to be securely available at our clinic.
